Appointment Requests

Requests submitted through our online form are not confirmed appointments. A member of our team will follow up by phone or email to schedule, verify insurance, and gather any additional information needed.

Payment and Insurance

Fees for services are communicated before treatment begins. We accept most major dental insurance plans and will help verify your benefits. You are responsible for any portion not covered by insurance. Accepted payment methods include cash, check, and major credit and debit cards. Online bill pay is available via the Pay My Bill link in our site header.

Cancellation

Please provide at least 24 hours notice when cancelling or rescheduling an appointment so the time can be offered to another patient. Repeated late cancellations or missed appointments may be subject to a fee.
